Coaches Dorian West - standing in for Nigel Redman - and Mark Mapletoft have made six changes to the line-up as they look at different combinations in midfield and in the front row in the build-up to the U-19 World Championships.
England beat Italy 18-11 last time out, with captain Hugo Ellis scoring both of his side's tries, but they are aiming to reverse last year's result against Wales when they lost 13-10 at The Stoop.
